Position Summary

Completing comprehensive evaluations of the client’s home and other environments (e.g., workplace, school), recommendations for adaptive equipment and training in its use, and guidance and education for family members and caregivers.

Qualifications

One year experience in completing comprehensive assessments
Excellent verbal and written communication skills and strong interpersonal skills
Knowledge of the regulatory requirements at the state, federal, and local level

Essential Functions

• An individualized evaluation, during which the client/family and occupational therapist determine the person’s goals,
• Customized intervention to improve the person’s ability to perform daily activities and reach the goals, and
• An outcomes evaluation to ensure that the goals are being met and/or make changes to the intervention plan.
• Develops and maintains data collection
• Provides leadership in the development of strategic long range of plans
• Ensure the accuracy of public information material and activates
• Make initial evaluation visit on all assigned patients within 24 hours of the first contact with the patient/family in order to:
• Determine whether the agency has the ability to adequately provide the necessary services in the home.
• Document for Client’s clinical record all care provided per Agency policy
• Participation in developing system or re-evaluating health care agency delivery of services.
• Participation in meeting requirements for state and health department survey for the agency.
• Ensure that regulatory documentation forms are completed and submitted in a timely manner.
