Description
POSITION SUMMARY: 
Home health aides ensure quality and safe delivery of home health care services and assist in the provision of patient care that reflects the Agency's philosophy and standards of health and nursing care of assigned patients. The Home Health Aide is supervised by the Nurse Care Manager.
 
QUALIFICATIONS:
  1. Experienced in home health care or related health care field.
  2. High School graduate or equivalent.
  3. Demonstrates ability to read, write, and carry out directions.
  4. Possesses maturity and ability to deal effectively with job demands.
  5. Good verbal and written communication skills.
  6. Minimum of 6 months of experience in Home health care.
  7. Possesses valid State Issued Photo Identification.
  8. Current CPR card
  9. Current First Aid card.
  10. Criminal background clearance.
  11. Reliable transportation.
  12. I-9 Documentation.
  13. Social Security Card.
  14. W-4 Federal Income Tax forms.
  15. State Tax forms.
  16. TB or Chest X-ray within 12 months prior to hire.

DUTIES:
  • Performs simple procedures as an extension of health care services under the direction and supervision of a Registered Nurse.
  • Perform personal care activities, including but not limited to:
    • Laundry
    • Bathing
    • Shampooing
    • Light housekeeping
    • Meal prep
    • Dressing
    • Shaving
    • Oral Hygiene
    • Skincare/nail care
  • Follows patient plan of care as written and discussed with the Nurse Care Manager. 
  • To ensure the quality and safe delivery of health care services.
  • Documentation to reflect the delivery of quality and safe health care services in the home.
  • Identifies and reports needs and changes in patient's condition or treatment.
  • Accurately reports to the Nurse Care Manager their findings and/or results in a timely manner.
  • Demonstrates the safe use of equipment and supplies effectively and efficiently.
  • Promotes personal safety and a safe environment for the client by observing infection control practices and reporting unsafe situations to the Nurse Care Manager. 
  • Participates in patient case conferences according to Agency policy and procedure.
  • Participates in education programs.
  • Complies with Agency Policy and Procedure.
 
WORK CONDITIONS:
  • As an "essential" employee may be called upon to assist with natural disasters and or public health emergencies.
  • Work involves exposure to hazardous conditions which may result in injury. 
  • Works with terminally ill patients.
  • Home visits may occur in unsafe neighborhoods and/or unhygienic environments.
  • Work may require use of protective equipment such as goggles, gloves, masks, etc. if patient has active TB or to protect AIDS patients from exposure to infection
