Description
QUALIFICATIONS:
- Experienced in home health care or related health care field.
- High School graduate or equivalent.
- Demonstrates ability to read, write, and carry out directions.
- Possesses maturity and ability to deal effectively with job demands.
- Good verbal and written communication skills.
- Minimum of 6 months of experience in Home health care.
- Possesses Maryland driver’s license and automobile insurance.
- Current CPR card/First Aid card
- Health clearance/physical within 12 months.
- Criminal background clearance.
- Reliable transportation.
- I-9 Documentation.
- Social Security.
- W-4 Federal Income Tax forms.
- State Tax forms.
- TB or Chest X-ray within 12 months.
- Hepatitis B series/declination.
- At least Two (2) letters of Reference (must be in good standing).
DUTIES:
- Performs simple procedures as an extension of health care services under the direction and supervision of a Registered Nurse.
- Perform personal care activities, including but not limited to:
- Bathing
- Shampooing
- Skincare/nail care
- Oral Hygiene
- Shaving
- Dressing
- Meal prep
- Light housekeeping
- Laundry
- To ensure the quality and safe delivery of health care services.
- Follows patient plan of care as written and discussed with the Registered Nurse.
- Documentation to reflect the delivery of quality and safe health care services in the home.
- Identifies and report needs and changes in patient’s condition or treatment.
- Accurately reports to the Registered Nurse their findings and/’or results in a timely manner.
- Demonstrates the safe use of equipments and supplies effectively and efficiently.
- Promotes personal safety and a safe environment for the client by observing infection control practices and reporting unsafe situations to the Registered Nurse.
- Participates in patient case conferences according to Agency policy and procedure.
- Participates in education programs.
- Complies with Agency Policy and Procedure.
- Must complete at least 12 in-service courses within 12 months
